** NOTE: As of August 2006, mentoring programs in Montana, Tennessee, or Virginia are no longer required to use special state-based procedures. If you are located in one of these three states, please follow the procedures outlined in the SafetyNET Pilot Program Manual above. This means that mentoring programs in all 50 states plus the District can now use the standard national SafetyNET system.

SafetyNET Forms
Several forms you will need for the SafetyNET pilot can be downloaded individually in both formats. These forms are also included in the Appendix of the SafetyNET Manual available above. Please make sure you read the SafetyNET Manual for full instructions on how to use these forms.

  • Volunteer Notice and Application Form - This form must be signed by your volunteers and submitted with each background check. PDF (29kb) | Word (87kb)

  • Checklist for Submitting a SafetyNET Background Check Request - This checklist helps ensure that you are sending all of the necessary documents to MENTOR when submitting an FBI criminal background check request. PDF (287kb) | Word (364kb)

  • Request for Release of Criminal History Information Notice and Form - This form is only used if a SafetyNET background check finds a criminal record. Volunteers would then use this form to request a complete copy of their criminal record from the FBI for themselves or for your organization.   • Credit Card Authorization Form - If you wish to pay for your SafetyNET background checks by credit card, please print out and complete this form. Then mail it in along with your background checks. Please note that credit card payments are subject to a $1.00 transaction fee. PDF (22kb) | Word (25kb)

  • Sample Fingerprint Card - This image shows what information must be filled into the various fields of the fingerprint card. Please use this as a reference so that your volunteer fingerprint cards are completed correctly. PDF (100kb) | Word (95kb)

  • Tips on Fingerprinting Volunteers - If you will be rolling the fingerprints of your volunteers, please read through these tips to ensure that your fingerprints are of high quality. Poor quality prints may be rejected by the FBI. PDF (298kb) | Word (572kb)
