SafetyNET Required Forms and Instructional Forms
Below are forms you will need to submit with each background check request. SafetyNET forms can be downloaded individually in Word or PDF format. All background check submissions with missing or incorrect data will be returned to the program. Please read the Program Manual for full instructions on how to use these forms.

• Checklist for Submitting a SafetyNET Background Check Request
  This checklist will help you ensure that you are sending all of the necessary documents to MENTOR when submitting an FBI criminal background check request. PDF (179kb) | Word (1.3mb)
  
  
• Volunteer Notice and Application Form
  This form must be signed by your volunteers and submitted with each background check. PDF (29kb) | Word (87kb)
  
  
• Check and Credit Card Payment Form
  This form must be completed in full whether you are paying via check or credit card. Please print the form and enclose it with your background check submission. To ensure we are able to process all SafetyNET requests efficiently, submissions without a payment form and exact payment amount will be returned. • Tips on Fingerprinting Volunteers
  If you will be rolling the fingerprints of your volunteers, please read carefully through these tips to ensure that your fingerprints are of high quality. Poor quality prints that cannot be read properly may be rejected by the FBI. If a print is rejected, the processing cost cannot be refunded. PDF (191kb) | Word (1.48mb)
  
  
• Sample Fingerprint Card
  This sample shows what information must be filled into the various fields of the fingerprint card. Please use this as a reference to ensure that all required information is complete and accurate before it is sent to MENTOR. Fingerprint submissions with missing or incorrect data will be returned to the program. Remember to also enclose a photocopy of a valid photo identification for each volunteer. • Request for Release of Criminal History Information Notice and Form
  This form is only used if a SafetyNET background check finds a criminal record. Volunteers may then use this form to request a complete copy of their criminal record from the FBI for themselves or for your organization.
